What is the CSM?

This Client Satisfaction Measurement (CSM) tracks the customer experience of government offices. Your feedback on your recently concluded transaction will help this office provide a better service. Personal information shared will be kept confidential and you always have the option to not answer this form.

1. Purpose of Data Collection

The Municipality of Sta. Rita, Pampanga collects your personal information solely for the purpose of measuring client satisfaction with government services, as required by ARTA. The data will be used to improve service delivery and generate mandated reports.

2. What Information We Collect

We may collect your name (optional), age, gender, the office you transacted with, and the service you availed. You may respond anonymously. Ratings and feedback are collected through the ARTA-prescribed CSM form.

3. How We Use Your Data

Your responses will be used to generate statistical reports for ARTA, produce office-level performance reports, and identify areas for service improvement. Individual responses are not disclosed publicly.

4. Data Retention & Security

All data is stored securely and accessible only to authorized personnel. Retention follows the National Archives of the Philippines regulations. Technical and organizational measures protect your data.

5. Your Rights

Under the Data Privacy Act of 2012, you have the right to be informed, to access your data, to object, and to request correction or deletion. Contact our Data Protection Officer at the Municipal Hall, Sta. Rita, Pampanga.
